Your Visit To Mission Gastroenterology

Not sure what you need to bring?

In order to help our staff serve you better, we ask that you review the list below. Our focus is to make your visit as efficient as possible.

Please bring the following items to your first appointment:

  • Print and complete forms downloaded from our website
  • Medications you are currently taking
  • Copies of X-rays, other imaging studies, or labs relevant to your visit
  • Copies of other medical records relevant to your visit
  • Your current insurance card and drivers license/photo identification
  • A method of payment for your co-pay, which is collected at registration Visa, MasterCard, American Express, Discover Card, personal checks and cash are accepted

Please arrive at least 20 to 30 minutes prior to your initial visit. This allows you time to fill out the necessary forms pertaining to your medical and personal information. You may download these forms on our patient forms page.

If you have previous medical records dealing with the reason you are being referred, please arrange to have those records forwarded to our office from your referring physician as early as possible. If your initial appointment is less than one week away, we recommend you bring copies of your medical record with you.

Failure to provide previous laboratory and x-ray results makes it difficult for Dr. Acosta to make accurate decisions about your care and may result in unnecessary delay in your evaluation.

At the time of each visit to our office, but especially during the first visit, please bring a list of medications you are taking, including any over-the-counter drugs and those prescribed by other physicians. It is important for your doctor to be aware of your medications, particularly to help prevent possible drug interactions.

Your family and friends are always welcome in our office. Your appointment is generally a private meeting between you and your physician, but you are certainly free to have a family member or close friend accompany you when you meet with the doctor.

Procedure visits do not allow for family members to be present in the procedure area.
